Your admission packet must be completed prior to your initial appointment. You must also submit a copy of a photo ID and insurance cards if applicable prior to the admission appointment.

Minors who are in the care of anyone other than their legal guardian are required to have proof of guardianship on file prior to appointment. You may email a copy of your photo ID and insurance card directly to contact@essential-grace.com
