My name is Kimberly Murphy and I am a board certified Psychiatric Nurse Practitioner. I believe there is no true benefit to the traditional fast-paced outpatient psychiatric environment and feel a patient care model should give more input to the treatment care plan where it belongs...the patient. Over time, I have been presented with potential processes that both work and were poorly designed, Through it all, I have garnered ongoing feedback from the patient population I have served over time. The most important aspect I learned above all is that most patients do not expect perfect but they do want to be listened to and actually heard when there are concerns about their treatment plan. Essential Grace Mental Health Services, LLC offers a range of services including evaluation, diagnosis, and medication management services. Services include medication management only. I no longer provide therapy services; however, a listing of local referral sources for therapy can be provided upon request.
Essential Grace Mental Health Services, LLC offers evaluation and treatment services BY APPOINTMENT ONLY. Virtual appointments are available for all Florida residents depending on insurance coverage.
The office is located at 6202 N 9th Ave, Suite 4 in Cordova Office Park. Parking is available in the front of the building.
FLORIDA RESIDENTS
Essential Grace Mental Health Services accepts Cigna (Evernorth), Aetna, Tricare, Humana, Medicare, United Healthcare (Optum), AvMed, Oscar Health, Florida Blue or Private Pay. Please be aware there are multiple plan options for Medicare and there is no guarantee your current plan will be accepted. Patients will need to contact their insurance provider to ensure their visit will be covered. If a copay is required it must be satisfied prior to or at the time of service. Patients will also need to contact their insurance provider to determine what if any copay will be applicable prior to the visit. If you require a prior authorization to be seen (PA or Referral) it is your responsibility to ensure this process is completed. The PA (referral) cannot originate from this office. Please contact your insurance company to verify whether you will need one and the process for completing this.
Please note at times Essential Grace Mental Health Services utilizes a billing company to allow patients to accept certain insurances. You will be notified if this is the case with your insurance and all billing processes will be required to be completed with that company.
The patient is responsible for all copays and fees not covered by insurance. Every effort is made to gain reimbursement from the patient's insurance company if utilizing insurance but unmet deductibles and insurance denials despite due diligence to bill insurance remain the patient's responsibility to pay outstanding balances.
Patients who are self pay may secure an appointment paid by cash, HSA, or debit/credit card in advance. Occasionally patients may utilize out of service network providers and submit a superbill to their company for reimbursement.

SELF PAY RATES ARE AS FOLLOWS:

$95.00 FOLLOW-UP EVALUATIONS
Fee-for-service providers are considered out-of-network for insurance companies. Under this model, patients are required to pay for all services up front and if out-of-network coverage is available the patient is responsible for working with the insurance company to determine whether some or all of the treatment fees are reimbursable. Patients will be provided with a superbill after each visit to present to the insurance company if applicable. You may download the Out of Network Benefits Worksheet on the Contact Us page.
Appointments should be kept as scheduled to ensure consistency in the treatment process. A $100.00 fee will be charged for cancellations without 24-hour notice or non-appearance for a scheduled visit. As a general rule, for medication management a patient must be seen at minimum every 3 months after medications are stabilized. Patients will be scheduled more frequently when attempting to stabilize on medications. All patients scheduled for a new appointment will receive an IPE (Initial Psychiatric Evaluation). The purpose of this initial evaluation is to determine whether Essential Grace Mental Health Services, LLC is able to provide the appropriate services a patient may need. The IPE does not guarantee continued services will continue if it is determined Essential Grace Mental Health Services, LLC cannot develop a proper treatment plan or be able to appropriately continue services for the patient.
